about this program available

at EMHS for those requiring

an extended stay.

Eligibility

· Enrollment in the Medicare Part A or a private program that insures skilled nursing facility days.

· Qualifying 3 day (3 midnights) hospital stay within the last 30 days.

· Require a skilled service that a patient is actively participating in the making progress toward established goals.

 

Qualifying skilled services include:

· Skilled nursing minimum 7 days/week.

· Physical or occupational therapy minimum 5 days/week

· Medication administration and adjustment.

· IV therapy

· Wounds needing daily skilled care.

 

Goal

At EMHS the ultimate goal of a Swing Bed stay is to provide the patient with an interdisciplinary team approach to allow for a safe discharge with improved functional abilities.

 

Introducing our Interdisciplinary Team:

 

Physical Therapy

The physical therapy member of the team address the physical mobility aspect of your needs.  A licensed, physical therapist will do an in-dept evaluation of your needs.  Once the evaluation is complete, goals are developed with you and your family to prepare you for your return to the community.

 

Occupational Therapy

The occupational therapist of the team will provide assessment and treatment of cognitive, sensory and physical disabilities.  The occupational therapist focuses on your self-care and daily living skills.  Your treatment plans are aimed at returning you to the highest level of independence in the least supervised setting.

 

Speech/Language Therapy

The speech language pathologist is available to provide help with communication and related disabilities.  The speech language pathologist is qualified to diagnose and treat speech, language and voice disorders, and swallowing problems.

 

Nursing

You will be assigned a primary nurse who will be responsible for planning, implementing, and coordinating your nursing care.

 

Clergy/Pastoral Care

This is available upon request.

 

Social Services

Social Services focuses on assisting you and your family in adapting to changes due to your illness or injury.  The social worker will monitor your progress and adjustments, and provide support and counseling.  Social Services will also provide assistance with discharge planning and coordinate referrals to community services.

 

Activities

Activities are used to help you use your leisure time in ways that enhance your health, independence and well-being.  We encourage you and your family to bring reading material, music, crafts, etc. that interest you.  We also have material on the activity cart for you.

 

Cardiopulmonary

The cardiopulmonary department at EMHS will be available to assist you in all your pulmonary needs.  We will deliver your respiratory medications and treatments as ordered by your physician.  A trained staff member will assess your needs and then develop a plan to aide you in living with your COPD, asthma, or other respiratory conditions.

 

Dietary

The Nutrition Services Department is under the direction of a registered dietitian and a certified dietary manager who is also certified in food service sanitation and safety.  Our staff prides itself in the service of wholesome home-style meals.  A representative of the department visits each patient daily to assure complete menu satisfaction.  Our menu is based on the new Food Guide Pyramid for optimal nutritional health.

 


 

Transition to Swing Bed from hospital status means incorporating the following:

 

· Minimum weekly visits from the doctor and as needed throughout stay.

· Increase participation in ADL’s (activities of daily living, i.e. self care, grooming and toileting).  Street clothes are encouraged instead of hospital gowns.

· Increase physical activity as recommended, such as getting out of bed for meals and ambulation.

· Involvement in managing medication, skin care, respiratory needs as appropriate.
